We rely on staff at aged care residences – Lifestyle Coordinators, Care Managers or other relevant aged care staff – to formally refer residents who are interested in documenting their life stories utilising our free biography service.

Please complete as many fields as possible in the referral form to help our assessor understand your resident. All information is kept strictly confidential in line with privacy legislation.
If you have not worked with Beyond Words at your location previously, please contact us for a briefing about the biography process, client suitability requirements and to ensure we have volunteer biographers in your area.

We encourage you to advise families of your referral to us. There is information for families here, including a flyer outlining how the program works.
It helps streamline our administration of your request if you complete the above online form, however if you prefer to complete a form manually, you can download it here and send it to the email address listed in the form. Note: manually emailing forms does cause delays to get your client into our system.
If you wish to refer more than one client at a time (by prior arrangement), you can enter the details into a spreadsheet which will save you time and allow us to populate our database.
A volunteer will be in touch with you (usually within 1-2 weeks) to arrange an initial meeting of your resident.
